All travelers outside the United States, including Canada, Bermuda, and the Caribbean, require a passport to re-enter the country.
The passport application requires two different fees and payment methods. Be sure to check the Passport Fees section for accepted payment methods.
Requirements for a New Passport
For complete information about requirements for a new passport, visit the U.S. Department of State website. Below is general information about apply for a new passport.
You must apply in person and complete Form DS-11 if any of the following apply:
- He never had an American passport.
- He is under 16 years old.
- Your present passport was issued more than 15 years ago.
- Your current passport is missing or stolen.
When applying for your passport, please present the following:
Proof of citizenship
Original or authorized copy of your birth certificate (must contain your parents’ full names).
Naturalization certificate (if available).
You must also provide a black-and-white photocopy of your proof of citizenship. We can provide this service for a fee of $1.00 per page.
Proof of identity (one of the following):
Valid driver’s license.
State identity card.
You will also need to provide a photocopy of the front and back of your ID. We can provide this service for a fee of $1.00 per page.
Temporary ID cards are not accepted as proof of identity.
Passport photo
Current color photograph (background must be completely white)
Front view – glasses must be removed
2 inches x 2 inches
The photo is available in the office for $15.00
From November 1, 2016, people applying for or renewing their passports will be required to remove their glasses for photography.
